Enter my vehicle in the four wheel drive trip listed below. All occupants of my vehicle will be registered. There will be at least one person in my vehicle 21 years of age or older who will be responsible for the occupants of my vehicle. I agree to abide by the rules of the Jamboree and guidance and instruction of the Adventure Committee. We will carry no pets, fireworks, firearms, sirens or trailers. All applications and waivers must be completed and signed by passengers. Incomplete applications will be returned. All reservations must be paid in advance. Refunds will be made if you are unable to attend, providing we are notified in writing six weeks prior to the event. A service charge of $40.00 per person will be made on all refunds. All open or soft top vehicles must have roll bars. Drivers must have liability insurance coverage on their vehicle to participate in the event selected. Off Road Adventures reserves to right to change trails, schedules, meals, etc. without prior notice due to weather conditions, permit requirements and/or vendor schedules.

RELEASE AND WAIVER OF LIABILITY AND INDEMNITY AGREEMENT
TO OFF-ROAD ADVERTISER AND EVENT SPONSORS
In consideration of being permitted to participate in any event sponsored, promoted or directed by Off-Road Advertiser, the undersigned for himself, his personal representatives, heirs, and next of kin, HEREBY RELEASES the Off-Road Advertiser magazine, its officers, directors, promoters, sponsors and employees and/or the Jamboree staff of all liability to the undersigned, his personal representatives, heirs, and next of kin for any loss or damage on account of injury to the undersigned of his property or resulting in the death of the undersigned whether caused by the negligent act or omission of the Releasees or otherwise while the undersigned is for any purpose participating in such event. It is fully understood by each of the undersigned that there is some inherent risk associated with this event, including damage to vehicles. In addition, the undersigned AGREES TO INDEMNIFY AND HOLD HARMLESS the Releasees from any loss, liability damage or cost they incur due to such participation by the undersigned, whether caused by Releasee's negligence or otherwise and AGREE TO ASSUME FULL RESPONSIBILITY AND RISK for any body injury, death or property damage from Releasee's negligence or otherwise while the undersigned hereby acknowledges and represent the following: (1) That he or she had read the foregoing Release and Waiver of Liability and Indemnity Agreement; (2) That he or she shall at all times, while riding in a vehicle participating in this event, wear their respective seatbelts, and (3) That the owner/driver certifies that he/she has inspected this vehicle and same is in good mechanical condition, and (4) That the owner/driver has informed himself about the event and the trail(s) involved, either by prior participation or by investigation into the event(s) and trail(s). WAIVER MUST BE SIGNED BY ALL PARTICIPANTS. For a participant under the age of 18 years of age, both parents, the parent with legal custody, or the minor's legal guardian must sign this release form and write the word "minor" and age next to the minor's name. Minors must have valid driver's license to participate in attendance prize drawings. I have read this release.
